I'm having the same probs with my card too. Some days it works fine, others only a few probs, and the rest just look horrible.

That looks like over heat. Busted Heatsink fan maybe? Try getting some prog to underclock

I used to get that, hasn't happened since I got my new card. In the short term, avoid minimizing your ffxi window. That seemed to cause problems for me.
